Breast implant exchange and revision in London

Breast implant revision is surgery to replace, remove or correct breast implants that were placed for cosmetic reasons. Mr Yildirim Oezdogan, consultant plastic surgeon, offers implant exchange and revision to adults at hospitals in London.

Reasons for revision

Common reasons include:

  • capsular contracture: firm, tight or painful scar tissue around the implant

  • implant rupture or leakage

  • change in implant position, rippling or asymmetry

  • wishing to change implant size, or to remove implants altogether

  • changes in breast shape over time, after pregnancy or weight change

  • concerns about implants, including symptoms some people attribute to them

What revision involves

Depending on your situation, surgery may include:

  • Implant exchange: replacing implants with new ones of a similar or different size

  • Capsulectomy: removing some or all of the scar capsule around the implant

  • Implant removal: with or without an uplift or fat transfer to restore shape

  • Pocket correction: repositioning the implant or changing its plane

The length of the operation depends on what is needed. A straightforward implant exchange usually takes 1–2 hours under general anaesthetic. When a partial or total capsulectomy is involved, it usually takes 2–3 hours under general anaesthetic, as a day case or with an overnight stay. If a capsulectomy is performed, a small drain may be placed to remove fluid. It usually stays in for a short time and is removed before or after you go home.

Your consultation

At your consultation Mr Oezdogan will:

  • discuss what you would like to change and why

  • examine you and explain which options are suitable, including non-surgical alternatives or not having surgery

  • explain the benefits, risks, recovery and the possibility of further surgery in the future

  • give you written information to take away

If you have them, please bring details of your current implants, such as your implant card or operation notes. An ultrasound or MRI scan may be recommended to check your implants before surgery.

Private consultations and surgery are offered to adults aged 18 and over. You will be given time to reflect before deciding, and a second consultation is offered before surgery is booked.

Risks and recovery

These are discussed in detail at your consultation and in writing. They include:

  • bleeding, infection and scarring

  • changes in nipple or breast sensation

  • recurrence of capsular contracture

  • asymmetry, rippling or implant position problems

  • loose or uneven skin after implant removal, which may need further surgery

  • for new implants: rupture, the need for future replacement and, rarely, breast implant-associated anaplastic large cell lymphoma (BIA-ALCL)

  • the possible need for further surgery

Most people return to desk work after 1–2 weeks and to strenuous exercise after about six weeks.

Surgery aims to improve shape and appearance, but individual results vary and no particular outcome can be guaranteed.

Where surgery takes place

Consultations and aesthetic breast surgery take place at Highgate Hospital (Nuffield Health) and The London Welbeck Hospital. Day-case procedures take place at Chase Lodge Hospital, Mill Hill.

Frequently asked questions

How do I know if my implant has ruptured?

Rupture often causes no symptoms. It may be found on an ultrasound or MRI scan, which can be arranged if needed.

Do implants need replacing after 10 years?

There is no fixed lifespan for implants. Replacement is considered when there is a problem or a change you would like.

How do I arrange a consultation?

Send an enquiry through the Contact page.

Do you treat patients under 18?

Private consultations and surgery are offered to adults aged 18 and over.

Can implants be removed without replacing them?

Yes. Removal can be combined with an uplift or fat transfer to improve shape.

I had my implants abroad. Can you help?

Yes. Please bring any documents you have about your implants and surgery.

Do I need a mammogram before surgery?

If you are 40 or over and have not had a mammogram in the past 12 months, a mammogram is recommended before surgery, in line with UK guidance on breast imaging before aesthetic breast surgery. Younger patients need imaging only if they have symptoms, a strong family history or other risk factors.
